Discharge planning begins at the time of your admission. Your physician will
inform you of your discharge and give you any necessary instructions about
caring for yourself after you leave.
We ask that you make transportation arrangements to leave the hospital as soon
as your discharge procedures are complete. A staff member or volunteer will be
available to escort you to the lobby by wheelchair.
Should you or your loved one require home health, hospice care or
physical therapy/rehabilitation, you have a variety of options from which to
choose. However, if you’ve enjoyed your care here at CTMC, we’re pleased
to inform you that we offer each of these services on an outpatient and in-home
basis as well.
